Cizik护理学院(CSON)使用E *交易价值ck student immunizations and certifications that can be shared with our clinical partners where students perform clinical assignments. All CSON students are required to upload and maintain current records while they are enrolled at the school. Failing to do so can delay, or in some cases, bar students from registering or maintaining enrollment in courses

何时访问E*值

Prior to accessing E*Value, please have the following information available:

• Your ApplyUTH/MyUTH User ID and password,

• All certifications required for your specific program such as BCLS, ACLS, RN license, etc.

• All items must be up to date from the start, to the end of the semester. Items that expire during the semester will not be accepted.
• Do not access E*Value on a mobile device or smart phone. You must be on a secure and stable Wi-Fi connection.

When all required immunization documents, licenses (if applicable), COVID-19 Attestation, and Consent for Limited Release Identifier forms have been uploaded into E*Value, submit a request throughCsonassistto have the records reviewed.. Submitting this form will place your file in a queue to be reviewed by the CSON Compliance Team. Files are reviewed in the order in which they are received. Average processing time is 10 business days; however, this can be longer during peak periods, including the first week of registration and as the start of the term approaches.

A: At this time, the COVID-19 vaccination is not required to enroll at UTHealth, however vaccination is strongly encouraged. In some instances, clinical sites and preceptors may require the vaccination to participate in clinical activities. If you are notified that a site requires the COVID-19 vaccination, you will be able to upload the information into E*Value. More information regarding vaccination availability for CSON students is available through theIntranet(must have IDM hold removed to access).

Q: Can I have my immunizations administered on campus?

Q: I have been accepted , but cannot access E*Value?

A: Please make sure that you have接受您的录取报价in ApplyUTH as this is required to generate access to E*Value. It can take up to two (2) weeks after accepting the offer to gain access to E*Value. If upon logging in to E*Value and there are no immunizations/certifications listed, simply logout and wait 24 hours before attempting again.

A:All clinical records must display patient name, birth date, and physician's name, as well as the clinic name and address. Please ensure that the record is current for the duration of the term. Immunizations that expire before the end of the semester will not be accepted.

Q: I submitted an initial records request, but my records were not complete – how do I request another review?

To assist with the processing of your request, please ensure that the email contains your Student ID and academic program/track. Please note that records must be uploaded directly into E*Value and cannot be emailed.

A: Yes, your CPR card must be current through the end of the semester for which you are attempting to enroll.
