Nurse Faculty Loan Program (NFLP) Forms

The following forms are applicable to students in the Nurse Faculty Loan Program (NFLP):

The following deferment form is to be used to defer loans administered by UTHealth.*

The UAS Financial Arrangement form will enable you to make financial arrangements for your student loan if you are delinquent or in default.

Cancellation Form

如果your financial situation meets the requirements for cancellation, as described on your promissory note, you may begin the cancellation process by completing the UAS Perkins Loan Cancellation form; only for canceling Perkins services.

Ensure your payments for your student loan(s) are received on-time for life of your loan; complete the UAS ACH Payment Plan form.*
