Thank you for scheduling an appointment at UTHealth Neurosciences – Texas Medical Center. This message contains information to help you prepare for your upcoming appointment.

We request you bring the following to your appointment:

  • Photo ID
  • Insurance and pharmacy card(s)
  • Any medical records

Directions

Our clinic is located in the Texas Medical Center. The clinic is on the 28th floor of the Memorial Hermann Medical Plaza, located at 6400 Fannin Street.

Convenient self-parking is available in the building’s parking garage, which is accessible from both Fannin Street and Main Street. The patient drop off lane is also accessible by turning right into the first entrance on Fannin Street.

Valet parking is available. To utilize valet parking, turn right into the second entrance on Fannin Street. You can also enter the valet area from Main Street, by turning at the Hermann Plaza Drive streetlight.

Once you park, ride the elevator in the parking garage to the first floor lobby. Then, ride to the 28th floor, using the elevator bank that services floors 25-30.

We also request that you bring any medical records you may have to your appointment, along with your insurance card(s) and photo ID.
