UTHealth Neurosciences – Greater Heights is on the 2nd floor of Medical Plaza 1 in the Memorial Hermann – Greater Heights Medical Complex, located at 1631 North Loop West.

Free self-parking is available in the parking garage located next door to Medical Plaza 1.

Once you park, ride the elevator to the 2nd floor of the parking garage and follow the crosswalk to Medical Plaza 1. The clinic is located to the right of the atrium, at the end of the hallway.

New Patient Registration

This secure site is where you will input your personal information and your medical history, so our clinicians can provide you with the best care possible. It is also where we communicate with our patients. Here, you can send a message to your provider, check lab results, request refills, and more.

All patients should complete the required forms by logging in to the MyUTHealth Portal before their appointment. Patients who do not complete the forms prior to their appointment may need to be rescheduled to avoid delays in care.

We also request that you bring any medical records you may have to your appointment, along with your insurance card(s) and photo ID.
