Requesting Religious Exemption:

Step 1: Please submit theHCPC COVID-19 Vaccination Exemption Request Form.

Step 2: Diversity & Equal Opportunity will review your request and provide you with notice of approval or denial of your request.

Requesting Medical Exemption:

Step 1: Please submit theHCPC COVID-19 Vaccination Exemption Request Form.

Step 2: Please submit theCOVID-19 Vaccine Medical Exemption Provider Statementto your provider and ask them to complete the form. The completed form should be returned to the Diversity and Equal Opportunity Office atCALL@uth.tmc.edu

Step 3: Diversity & Equal Opportunity will review your request and your medical provider’s statement and then provide you with notice of approval or denial of your request.
